This Telehealth Consent explains how telehealth services are provided through WarriorBabe and what it means for you to receive care using remote technologies. By checking the acknowledgment box, initialing where requested, signing electronically, or otherwise indicating acceptance, you agree to the terms of this Telehealth Consent.
For purposes of this Consent, “WarriorBabe,” “we,” “us,” and “our” refer to Alpha Female Fitness, LLC d/b/a WarriorBabe and WarriorBabe Health, LLC, as applicable, and to the telehealth platform, intake, scheduling, communication, and support services made available under the WarriorBabe brand. Clinical telehealth services are provided by licensed clinicians practicing through Dynamic Health Institute LLC, doing business as Dynamic Health Institute, and any other affiliated medical groups or licensed providers engaged to furnish medical care through the WarriorBabe experience.
Telehealth, also referred to as telemedicine or virtual care, involves the use of secure electronic communications and information technologies to enable health care when you and your clinician are not in the same physical location. Telehealth services may include live video visits, audio-only calls when clinically appropriate, secure messaging, remote review of medical records, lab results, diagnostic information, photos, questionnaires, and follow-up communications related to your care.
Telehealth services offered through the WarriorBabe experience may include evaluation, diagnosis, treatment planning, monitoring, follow-up, and care coordination related to hormone optimization, metabolic health, weight management, preventative wellness, performance, longevity, and other services permitted by law and by clinician judgment.
WarriorBabe is the front-facing brand and patient experience through which you may access education, intake, scheduling, communications, and support services. Medical care, diagnoses, treatment decisions, prescriptions, and clinical recommendations are provided by licensed clinicians practicing through Dynamic Health Institute LLC or other affiliated medical groups, not by WarriorBabe coaching staff, customer support, or non-clinical personnel.
You understand and agree that:
• WarriorBabe may facilitate access to telehealth services, scheduling, communication, and coordination.
• Clinical services are rendered by licensed medical professionals who are responsible for medical judgment and clinical decision-making.
• Not every service offered under the broader WarriorBabe brand is a medical service, and not every member of the WarriorBabe team is a licensed medical provider.
Potential benefits of telehealth include improved access to care, reduced travel time, increased convenience, continuity of care, and the ability to receive specialized support from home or another remote location. Telehealth may also support more efficient follow-up care, review of lab results, medication management, and care planning between visits.
Telehealth also has limitations and risks. These may include:
• Delays caused by equipment failure, connectivity problems, poor audio or video quality, or other technical issues.
• A more limited ability to perform a physical examination, take vital signs directly, or observe certain clinical findings that might be easier to assess in person.
• Risks to privacy or confidentiality if security is compromised, if you use an insecure connection, or if someone else can overhear or view your visit on your side.
• The possibility that telehealth may not be appropriate for your condition, symptoms, or clinical needs.
You understand that no technology system can guarantee absolute privacy or uninterrupted performance, although reasonable administrative, technical, and physical safeguards are used to protect your information.
Alternatives to telehealth may include in-person care, evaluation by your local primary care or specialist provider, urgent care, emergency room services, or other healthcare settings depending on the nature of your condition. Your clinician may determine that telehealth is not clinically appropriate for some or all of your needs and may require you to seek in-person care before treatment can continue.
Telehealth services made available through WarriorBabe are not intended for emergency care. If you are experiencing a medical emergency, call 911 immediately or go to the nearest emergency room. If you are experiencing a mental health crisis, thoughts of self-harm, or thoughts of harming others, call 988 or seek emergency assistance immediately.
You understand that messages sent through forms, email, portal messaging, or other WarriorBabe communication channels may not be reviewed in real time and should not be used for emergencies or time-sensitive medical issues.
To help protect your privacy and support safe, effective telehealth care, you agree to:
• Provide complete, accurate, and current information about your symptoms, health history, medications, supplements, allergies, and other relevant matters.
• Participate in visits from a location where you can communicate privately and safely whenever possible.
• Use a reliable internet connection, device, and contact information to the best of your ability.
• Follow clinician instructions regarding labs, imaging, referrals, in-person evaluations, medication use, and follow-up appointments.
• Inform your clinician of the state where you are physically located at the time of each telehealth encounter, because services may only be provided where your clinician is licensed or otherwise authorized to practice.
You understand that inaccurate or incomplete information may limit your clinician’s ability to diagnose, treat, or monitor your condition safely.
Your information, including protected health information, may be collected, used, shared, stored, and protected as described in the WarriorBabe Privacy Policy and WarriorBabe Notice of Privacy Practices (HIPAA Notice), which are incorporated into this Consent by reference. This may include sharing information with clinicians, labs, pharmacies, payment vendors, telehealth platform providers, and other service providers involved in your care, operations, and payment processing, as permitted by law.
By accepting this Consent, you acknowledge that you have been given access to the WarriorBabe Privacy Policy and Notice of Privacy Practices and understand that those documents apply to telehealth services as well.
Telehealth visits will not be recorded by WarriorBabe or its affiliated clinical entities unless specifically disclosed to you in advance and authorized where required by law. You may not audio record, video record, photograph, or otherwise capture telehealth visits without express written permission from WarriorBabe and the treating clinician, unless applicable law gives you that right.
You agree that operational and care-related communications may be delivered electronically, including through email, SMS, phone, and secure portal or app messaging, subject to your communication preferences and applicable law.
Telehealth laws vary based on the patient’s location and the provider’s licensure status. You understand that telehealth services are only available when you are physically present in a state or jurisdiction where the treating clinician is licensed or otherwise legally authorized to provide care. Because practitioner availability and licensure vary over time, WarriorBabe does not guarantee service availability in every state at every time.
If you move, travel, or change location before or during treatment, you agree to notify the care team so that eligibility for continued telehealth services can be confirmed.
Depending on your needs, telehealth services may involve lab testing, pharmacy coordination, prescriptions, supplement recommendations, monitoring, or referrals. You understand that:
• A clinician may require laboratory testing, medical records, imaging, or other information before diagnosis or treatment.
• Prescriptions may be issued only when clinically appropriate and legally permissible.
• Certain medications, therapies, or treatments may require in-person evaluation, ongoing monitoring, or additional documentation.
No specific outcome, diagnosis, prescription, or treatment plan is guaranteed.
You understand that telehealth services may be billed to you directly, billed to your health plan when applicable, or provided on a self-pay basis depending on the service model. You are responsible for charges not covered by insurance and for self-pay services, memberships, labs, medications, supplements, or follow-up care that are not included in a covered benefit or stated package.
Additional billing, cancellation, and payment terms may be described in the WarriorBabe Terms of Service, checkout terms, or Financial Responsibility and Payment Authorization documents, where applicable.
You may withdraw this Telehealth Consent for future care at any time by notifying WarriorBabe or the clinical care team in writing using the contact information below. Withdrawal of consent will not affect:
• Services already provided in reliance on this Consent.
• Uses or disclosures of information already made as permitted by law.
• Any obligation to pay for services already rendered.
If you withdraw consent, telehealth services may no longer be available to you unless and until a new consent is completed.

• You have had the opportunity to review the WarriorBabe Privacy Policy and Notice of Privacy Practices.
• You understand the benefits, risks, and limitations of telehealth services.
• You understand that WarriorBabe is the patient-facing brand and that clinical services are provided by licensed clinicians practicing through Dynamic Health Institute LLC or other affiliated medical groups.
• You voluntarily consent to receive telehealth services through WarriorBabe under the terms described in this document.
