This Telehealth Informed Consent (“Consent”) describes the use of telehealth services available through WarriorBabe Health, LLC (“WarriorBabe Health,” “we,” “our”). By proceeding with an intake, consultation, or treatment plan through the Platform, you consent to receive healthcare services via telehealth.
1. What Telehealth Is
“Telehealth” means the delivery of healthcare services using electronic communications, information technology, or other means between a patient and a provider who are not in the same physical location.
Telehealth may include:
- Intake forms and health questionnaires
- Secure messaging, chat, or video visits
- Electronic prescription and pharmacy services
- Review of medical records and test results
2. Potential Benefits
- Convenience and accessibility from your home
- Expanded access to licensed providers across states
- Continuity of care and faster response times
3. Potential Risks
- Technical failures (poor internet, dropped calls, delays)
- Limited ability to perform a physical exam
- Rare but possible unauthorized access despite security safeguards
- Telehealth may not be appropriate for all conditions
4. Alternatives
You may seek in-person care from a local provider instead of or in addition to telehealth. Providers may refer you to in-person care if they believe telehealth is not appropriate for your condition.
5. Emergencies
Telehealth is not for emergencies. If you believe you are experiencing a medical emergency, call 911 immediately.
6. Privacy & Confidentiality
- WarriorBabe Health is not a HIPAA-covered entity.
- Clinical services are provided by Beluga Health, a licensed telehealth medical group.
- Prescriptions, if appropriate, are fulfilled by ChemistryRX, a licensed U.S. pharmacy.
- Beluga and ChemistryRX may be HIPAA-covered entities and may provide their own Notices of Privacy Practices.
- We use reasonable administrative, technical, and physical safeguards to protect your information. For more details, see our Privacy Policy.
7. State Eligibility
Telehealth services are available only in select states. You must be physically located in an eligible state at the time of your visit. Availability may change.
8. Patient Rights
- You may withdraw consent to telehealth at any time by stopping use of the Platform and notifying support.
- You may request in-person care at any time.
- You have the right to access your medical records as permitted by law.
- You have the right to know the credentials of your provider.
9. Provider & Pharmacy Disclosure
- All medical services are provided by Beluga Health (belugahealth.com).
- Prescriptions, if appropriate, are filled and shipped by ChemistryRX (chemistryrx.com), a licensed U.S. pharmacy accredited by URAC.
10. Consent
By clicking “I Agree,” “Submit,” or proceeding with care on the WarriorBabe Health Platform, you acknowledge that you have read, understood, and agree to this Telehealth Informed Consent. You understand the risks, benefits, and alternatives of telehealth and consent to receive care in this manner.
