What standard panels miss

Your physical might run a CBC, a basic metabolic panel, a lipid screen, and a TSH. That’s <15 markers, often interpreted against ranges built for the average 30-year-old.

Estradiol, progesterone, free testosterone, SHBG, DHEA, free T3, reverse T3, fasting insulin, hs-CRP, ferritin, B12, vitamin D, homocysteine, ApoB. Almost none of these get pulled in a routine visit.

What “normal” really means

Lab reference ranges are statistical. They're built from the population that walked into the lab to get tested. That population is mostly sick.

You don’t want to be in the middle of that distribution. You want to be in the optimal window for a healthy, active woman in your 40s or 50s. Those are different ranges. We use both.

Specific markers vary by your symptoms, history, and which protocol you’re entering. Here’s the framework.

Sex hormones

Estradiol, progesterone, total and free testosterone, SHBG, DHEA-S, FSH, LH. The full picture, timed where the cycle still applies.

Comprehensive thyroid

TSH, free T3, free T4, reverse T3, TPO antibodies. Most physicians run only TSH. You can have full-blown thyroid dysfunction with a normal TSH.

Metabolic and insulin

Fasting glucose, fasting insulin, HOMA-IR, HbA1c. Insulin resistance hides for a decade before diabetes shows up, and it drives perimenopausal weight gain.

Inflammation and immune

hs-CRP, homocysteine, ferritin (also iron status), full CBC. Chronic low-grade inflammation drives mood, joint pain, and metabolic dysfunction.

Micronutrient status

Vitamin D, B12, folate, magnesium where available. Deficiency in any of these can mimic hormone symptoms. We rule it out before treating.

Cardiovascular detail

Standard lipid panel plus ApoB and Lp(a) when indicated. The standard cholesterol panel misses most of what actually predicts heart disease in women.

How is this different from my annual physical labs?

Your physical typically runs 10 to 15 markers focused on disease screening. Our panel runs 50+ markers focused on hormone, metabolic, and inflammatory function, and is read against optimal ranges, not just standard reference ranges.

Will my insurance cover the labs?

Often yes. We help you check eligibility before ordering, and many clients pay little or nothing out of pocket for the labs. If your insurance doesn't cover the panel, our cash-pay rates are typically a fraction of retail. HSA / FSA cards work everywhere.

Where do I get drawn?

We work with a nationwide network of labs. You can walk into one near you, or schedule at-home phlebotomy in many areas. Your closest options show up once your panel is ordered.

Do I have to fast?

For accurate metabolic markers (insulin, glucose, lipids), yes. Typically a 10 to 12 hour overnight fast. We send you full prep instructions before your draw.

How often should I retest?

For most clients on an active protocol: a 6-week recheck after starting, then every 6 months. Big life changes (peri-to-meno transition, surgery, major weight change) warrant a re-pull.

Do I need this if I’m already on HRT or supplements elsewhere?

If you don’t know your numbers, you’re flying blind. Many women come to us already on HRT and discover their dose is wrong, their thyroid was missed, or their inflammation markers are off. Then we fix it.
