This Notice of Privacy Practices (“Notice”) describes how WarriorBabe Health, LLC and its affiliated clinical entities, including Dynamic Health Institute, may use and disclose your protected health information (“PHI”), and what rights you have with respect to that information under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and applicable state law.
For purposes of this Notice, “we,” “us,” and “our” refer to WarriorBabe Health, its telehealth operations, and Dynamic Health Institute and other affiliated medical groups that provide clinical services to you through the WarriorBabe Health platform.
We are required by law to:
• Maintain the privacy and security of your PHI.
• Provide you with this Notice describing our legal duties and privacy practices.
• Follow the terms of the Notice currently in effect.
• Notify you following a breach of unsecured PHI when required by law.
We may use and disclose your PHI for the following purposes, as permitted by HIPAA and state law:
a. Treatment
We may use and share your PHI to provide, coordinate, or manage your health care and related services. This includes:
• Sharing information with WarriorBabe Health clinicians and care teams.
• Sending information to labs, pharmacies, or other providers involved in your care.
b. Payment
We may use and disclose your PHI to obtain payment for services, including:
• Submitting claims to your health plan and responding to plan requests.
• Determining eligibility, coverage, and benefits.
• Billing you directly for services that are not covered or are self-pay.
c. Health Care Operations
We may use and disclose PHI for our operations, such as:
• Quality assessment and improvement activities.
• Training, credentialing, and performance review of clinicians and staff.
• Internal audits, compliance programs, and legal/risk management.
• Business planning and management.
d. Business Associates
We may share PHI with third-party service providers (for example, telehealth platform vendors, cloud providers, billing and payment vendors, or analytics tools) that perform services on our behalf and that have agreed in writing to protect your information as our “business associates.”
e. As Required by Law
We may use or disclose PHI when required to do so by federal, state, or local law.
f. Public Health and Safety
We may share PHI for public health and safety purposes, such as:
• Reporting certain diseases or injuries.
• Reporting adverse events, product defects, or recalls.
• Reporting suspected abuse, neglect, or domestic violence as required by law.
• Preventing or reducing a serious and imminent threat to a person’s or the public’s health or safety.
g. Health Oversight Activities
We may disclose PHI to health oversight agencies for activities authorized by law, such as audits, inspections, investigations, or licensure actions.
h. Judicial and Administrative Proceedings
We may disclose PHI in response to a court or administrative order, subpoena, discovery request, or other lawful process, subject to applicable protections.
i. Law Enforcement
We may disclose PHI for certain law enforcement purposes, such as responding to lawful requests for information to locate a suspect, material witness, or missing person, or to report crimes in limited circumstances.
j. Specialized Government Functions
When required, we may disclose PHI for specialized government functions, such as military or national security activities, or for correctional institutions.
k. Workers’ Compensation
We may disclose PHI as authorized by workers’ compensation or similar laws that provide benefits for work-related injuries or illness.
l. Limited Data Sets and De-identified Information
We may use and disclose PHI that has been de-identified or converted to a limited data set (with direct identifiers removed) for research, analytics, or health care operations, as permitted by law.
Certain uses and disclosures of your PHI require your prior written authorization. Examples include:
• Most uses and disclosures of psychotherapy notes (where applicable).
• Certain uses and disclosures of PHI for marketing purposes.
• Sales of PHI as defined by HIPAA.
If you provide authorization for a specific use or disclosure, you may revoke that authorization at any time in writing, except to the extent we have already acted in reliance on it.
You have the following rights regarding PHI we maintain about you, subject to certain limitations and exceptions:
a. Right to Access and Obtain Copies
You have the right to request access to and copies of your PHI in our designated record set. This may include the ability to receive an electronic copy where we maintain records electronically.
b. Right to Request Amendments
If you believe information about you is incorrect or incomplete, you may request that we amend your PHI. We may deny your request in certain circumstances, but we will tell you why in writing and explain your rights.
c. Right to an Accounting of Disclosures
You may request an accounting of certain disclosures of your PHI made in the prior six years, excluding disclosures for treatment, payment, and health care operations and certain other exceptions.
d. Right to Request Restrictions
You may request restrictions on how we use or disclose your PHI for treatment, payment, or health care operations. We are not required to agree to all requested restrictions, except in some cases where you pay out-of-pocket in full and request that we not share that information with your health plan, if legally allowed.
e. Right to Request Confidential Communications
You may request that we communicate with you about your PHI in a specific way (for example, by email only, or at a different address) and we will accommodate reasonable requests.
f. Right to a Paper or Electronic Copy of This Notice
You may request a paper copy of this Notice at any time, even if you have agreed to receive it electronically.
To exercise any of these rights, please contact us using the information in the “Contact Us” section below. We may ask you to submit certain requests in writing.
Because we provide services through telehealth, we may collect, use, and disclose PHI through electronic means such as secure video, phone, messaging, and data exchange with labs and pharmacies. We use reasonable safeguards to protect PHI in telehealth, but as with any technology, there are inherent risks of unauthorized access despite our efforts.

We reserve the right to change our privacy practices and this Notice at any time, as permitted by law. Changes will apply to PHI we already have about you as well as any new PHI we receive in the future.
When we make material changes, we will update the “Last updated” date at the top of this Notice and make the revised Notice available through our website and/or patient portal.
If you believe your privacy rights have been violated, you may file a complaint with us and/or with the U.S. Department of Health and Human Services, Office for Civil Rights.
We will not retaliate against you for filing a complaint.
